Please ensure your child brings the following, signed consent form to the vaccine clinic. If you are a parent to a child under 17 and will not be accompanying your child to the vaccine clinic, please print this form and sign the second page. Consent form MUST be legible and signed in order to have a digital record from the state.

Must bring your vaccine card (Copy or screen shot acceptable) or digital vaccine record

Pre-Registration is no longer required. In order to test or receive a vaccination, please visit our nearest testing location at your convenience.

Our goal is to help all students and staff return to school safely and with peace of mind.

*Inspire Diagnostics will bill your health plan, the government, a government health plan or the uninsured fund for the vaccine. There will be no patient out-of-pocket cost.
